Fruquintinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beet interacts with.

Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.

Here's the idea behind this pairing. Fruquintinib (Fruzaqla) is cleared from your body partly by a liver enzyme called CYP3A4. Some lab research suggests that betanin, the pigment that makes beets red, may slow that enzyme down. If that happened in real life, your body would clear fruquintinib more slowly, and the drug could build up. That could mean stronger side effects like high blood pressure, mouth sores, hand-foot skin reactions, fatigue, or bleeding.

But I want to be honest about the strength of this. This is only theoretical, based on test-tube studies, not on people taking beets. Everyday beets in your food are very unlikely to cause a problem. If you're considering concentrated beetroot supplements while on fruquintinib, please check with your oncology team or pharmacist first.

What Is Fruquintinib Used For?

Brand name: Fruzaqla

Fruzaqla (fruquintinib) is used to treat adults with metastatic colorectal cancer (mCRC) — colorectal cancer that has spread to other parts of the body. It is specifically for patients who have already been treated with several standard chemotherapy regimens and other targeted therapies.

Fruzaqla is intended for patients who have previously received fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and — if their tumor is RAS wild-type and it is medically appropriate — an anti-EGFR therapy.
