Beet and Remdesivir

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Remdesivir

Remdesivir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beet interacts with.

Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: beets contain a pigment called betanin, and in lab dish studies it slowed down an enzyme called CYP3A4. Remdesivir is partly handled by that same enzyme. So in theory, large concentrated beet doses could slow how your body clears remdesivir, letting levels run a little higher. That could in turn nudge up the chance of side effects like nausea or changes in liver enzymes.

I want to be honest about how thin this evidence is. It comes only from test-tube work, not from people, so we really don't know if it matters in real life. Remdesivir is also given in the hospital or clinic by IV under close watch. Food-amounts of beets are very unlikely to cause trouble. If you take beetroot supplements, just mention it to your care team.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Remdesivir page

“Remdesivir (Veklury) is an IV antiviral for serious COVID-19, and people receiving it have often spent the prior week loading up on elderberry and other immune boosters, a habit that deserves a hard look here. Seven of the 140 supplement ingredients on file are rated major, and those seven are the ones to understand first.”
