Green Tea and Remdesivir

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Green Tea interacts with whole categories of medication. Remdesivir falls into 2 of those categories, so it can interact with Green Tea in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

P-GLYCOPROTEIN SUBSTRATES

1 of 2

Why this applies to Remdesivir

Remdesivir falls under P-GLYCOPROTEIN SUBSTRATES — the group Green Tea interacts with.

Green tea might increase the levels and adverse effects of P-glycoprotein (P-gp) substrates.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the practical side of this one. Remdesivir is moved around the body partly by a transporter called P-glycoprotein, which acts like a little pump that helps push the drug along. Green tea, at least in lab studies and a handful of case reports, may slow that pump down. In theory, that could let a bit more remdesivir build up, which might increase the chance of side effects like nausea or changes in liver enzymes.

That said, this is weak, anecdotal evidence, not something proven in real patients on this exact drug. Also worth knowing: remdesivir is given by IV in a medical setting, so your care team is already watching your labs closely. Just mention any green tea supplements you take to your pharmacist or doctor so they're in the loop.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Remdesivir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: this is a very low-worry combination. Remdesivir is processed in part by a liver enzyme called CYP3A4, and lab studies suggest green tea can nudge that enzyme in both directions (slowing it in the gut, speeding it up a bit in the liver). On paper that could change remdesivir levels, but in actual human studies green tea doesn't seem to meaningfully affect this enzyme.

On top of that, remdesivir is given as an IV infusion in a hospital or clinic, where your care team monitors you closely. I wouldn't expect your morning cup of green tea to change how well remdesivir works or cause extra side effects. If you take concentrated green tea extract pills, just mention it to your doctor or pharmacist so they have the full picture.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Remdesivir page

“Remdesivir (Veklury) is an IV antiviral for serious COVID-19, and people receiving it have often spent the prior week loading up on elderberry and other immune boosters, a habit that deserves a hard look here. Seven of the 140 supplement ingredients on file are rated major, and those seven are the ones to understand first.”

About Green Tea

What is Green Tea?

Green Tea
  • Antioxidant support
  • Weight loss
  • Heart health
  • Mental alertness
  • Cancer risk reduction

Green tea is a popular beverage rich in antioxidants called catechins, and drinking it in normal amounts is considered safe for most people. Concentrated green tea extracts are a different story—they may help modestly with weight or heart health for some, but high doses have been linked to rare liver problems. Talk to your pharmacist or doctor before taking green tea supplements, especially if you have liver concerns or are pregnant.
