Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the thing with Evotaz (atazanavir plus cobicistat): your body clears atazanavir using a liver enzyme called CYP3A4. Berberine appears to slow that same enzyme down a bit. So in theory, adding berberine could let atazanavir build up a little higher than expected, which may raise the chance of side effects like nausea, headache, or that yellowing of the eyes or skin (jaundice) atazanavir is known for. The cobicistat in this combo is itself a strong booster, so there's already a lot going on with this enzyme.

I'll be honest, this concern is mostly theoretical and not well studied in people taking HIV medicine. Still, because keeping your HIV treatment steady matters so much, please don't start berberine without first running it by your doctor or pharmacist.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
