Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the situation with these two. Your body clears bedaquiline (Sirturo) mainly through a liver enzyme called CYP3A4. Berberine appears to slow that same enzyme down. So in theory, taking them together could let bedaquiline build up a bit higher in your blood than expected.

Why does that matter? Bedaquiline can affect your heart's rhythm (a change called QT prolongation), and higher levels may raise that risk, along with possible liver irritation or nausea. I'll be honest, this is mostly a theoretical concern based on lab and early research, not proven harm in people taking both. Still, TB treatment is too important to gamble with. Please don't start berberine without first checking with your doctor or pharmacist managing your TB care.

About Bedaquiline

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
