Berberine and Belladonna, Caffeine, Ergotamine, Pentobarbital

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Berberine interacts with whole categories of medication. Belladonna, Caffeine, Ergotamine, Pentobarbital falls into 3 of those categories, so it can interact with Berberine in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CNS DEPRESSANTS

1 of 3

Why this applies to Belladonna, Caffeine, Ergotamine, Pentobarbital

Belladonna, Caffeine, Ergotamine, Pentobarbital falls under CNS DEPRESSANTS — the group Berberine interacts with.

Theoretically, berberine might increase the sedative effects of CNS depressants.
Animal research suggests that berberine may have sedative effects. Theoretically, use of berberine along with CNS depressants might produce additive therapeutic and adverse effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination product contains pentobarbital, a barbiturate that calms the nervous system, which is the part that matters here. Berberine has shown mild sedative effects in animal studies, so on paper, pairing it with a sedating medicine like this could add up. You might feel extra drowsy, foggy, lightheaded, or more relaxed than usual.

I want to be honest about the evidence though: this is based only on lab and animal research, not human studies, so it's more theoretical than proven. Still, since this medicine already includes a barbiturate and ergotamine, it's worth a quick chat with your pharmacist or doctor before adding berberine. Don't stop or change anything on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 3

Belladonna, Caffeine, Ergotamine, Pentobarbital also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination product contains several active ingredients, but the one I watch most closely here is ergotamine. Your liver clears ergotamine using an enzyme called CYP3A4, and berberine appears to slow that enzyme down. In plain terms, berberine could let ergotamine build up more than expected, which raises the chance of side effects like nausea, tingling or coldness in your fingers and toes, leg cramps, or in rare cases reduced blood flow to your hands and feet. The caffeine in this product may also linger a bit longer, leaving you jittery or sleepless.

To be fair, this is based mostly on lab work and early studies, so it's a possible concern, not a sure thing. Still, please run it by your pharmacist or doctor before adding berberine.

PENTOBARBITAL (Nembutal)

3 of 3
Berberine might increase the sedative effect of pentobarbital.
Evidence from animal research shows that berberine can prolong pentobarbital-induced sleeping time. Theoretically, combining berberine and pentobarbital might increase the sedative effects of pentobarbital.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

The combination product you're asking about contains pentobarbital, a barbiturate that calms the nervous system. That's the piece that matters here. In animal studies, berberine made pentobarbital's sleep-inducing effect last longer, so in theory the two together could leave you feeling more drowsy, groggy, or sluggish than the medicine alone.

I want to be honest about the evidence though. This is based on lab and animal research, not solid human studies, so it's a possible concern rather than a proven one. Still, since this product already has sedating ingredients, I'd play it safe. Before adding berberine, have a quick chat with your pharmacist or doctor, especially if you notice extra sleepiness, dizziness, or slowed thinking.
