Theoretically, berberine might have additive effects with antihypertensive drugs.
Animal research suggests that berberine can have hypotensive effects. Also, a clinical study suggests that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone.

Here's the thing to keep in mind if you're thinking about adding berberine while taking bosentan (Tracleer). Berberine has shown some blood pressure lowering effects in animal studies, and one human study found it added to amlodipine's effect. Bosentan isn't a typical blood pressure pill, but it does relax the vessels in your lungs, so combining the two could nudge your blood pressure a little lower than expected.

In real life that might show up as dizziness, lightheadedness, or feeling faint, especially when you stand up. To be honest, this concern is mostly theoretical and based on lab and animal work, so it's a gentle caution rather than a red flag. Still, please check with your pharmacist or doctor before starting berberine so they can keep an eye on your readings.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP2C9.
Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, taking berberine with drugs metabolized by CYP2C9 might increase drug levels and increase the risk of adverse effects.

Here's the gist for Bosentan (Tracleer): your liver uses an enzyme called CYP2C9 to help clear this medicine from your body. Berberine appears to slow that enzyme down a bit, so in theory more bosentan could linger in your bloodstream. Bosentan is active on its own (not a prodrug), so higher levels would mean stronger effects and a greater chance of side effects like headache, flushing, swelling in the legs, or changes in your liver blood tests.

I want to be honest, though: this is mostly theoretical and based on early, lower-quality research, so it may not amount to much in real life. Still, because bosentan already needs liver monitoring, please loop in your pharmacist or doctor before adding berberine rather than starting it on your own.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
