Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the situation with berberine and your caffeine product. Berberine appears to mildly slow down a liver enzyme called CYP3A4, which is one of the systems your body uses to clear certain drugs. If less of the caffeine is broken down, more of it can hang around, so you may feel its effects more strongly. Think jitteriness, a racing or pounding heart, trouble sleeping, anxious feelings, or an upset stomach.

That said, much of this comes from lab and early studies, so it's more of a theoretical caution than a guarantee. Caffeine itself is not a prodrug, meaning it doesn't need this enzyme to switch on. If you notice you feel wired or wound up, mention it to your pharmacist or doctor before changing anything.
