Cariprazine Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the gist with Vraylar (cariprazine) and berberine. Your body clears cariprazine using a liver enzyme called CYP3A4. Berberine appears to slow that enzyme down a bit, so the worry is that cariprazine could build up higher than intended in your system. Cariprazine is also a long-acting drug that lingers for weeks, so even a modest bump can add up over time.

If levels climb, you might notice more of its usual side effects, things like restlessness or trouble sitting still, sleepiness, dizziness, or stiffness. That said, this is largely theoretical and based on early lab and limited human data, so it may not cause real trouble for everyone. Please don't change anything on your own. Just check with your pharmacist or doctor before adding berberine.

What Is Cariprazine Used For?

Brand name: Vraylar

Vraylar (cariprazine) is approved to treat schizophrenia in adults and in adolescents aged 13 and older. It is also used for the acute treatment of manic or mixed episodes associated with bipolar I disorder in adults and in children aged 10 and older.

In adults, Vraylar is additionally approved to treat depressive episodes associated with bipolar I disorder (bipolar depression) and as an add-on (adjunctive) therapy alongside antidepressants for major depressive disorder (MDD).
