Here's the quick version: defibrotide is a clot-fighting medicine given by IV in the hospital, usually for a serious liver complication after a stem-cell transplant. Because it already works to keep blood from clotting, adding berberine on top could, in theory, nudge the bleeding risk a little higher.

I want to be honest about how strong this concern is. The worry comes from lab and animal studies showing berberine can make platelets less sticky. We don't have solid human evidence that it actually causes problems with defibrotide. Still, since defibrotide is only used in very sick, closely monitored patients, the care team watches carefully for signs of bleeding like easy bruising, nosebleeds, or blood in the stool. Always let the doctors and pharmacist know about any supplement before or during this treatment.

What Is Defibrotide Injection Used For?

Brand name: Defitelio

Defitelio (defibrotide injection) is used to treat hepatic veno-occlusive disease (VOD) — also called sinusoidal obstruction syndrome (SOS) — in adult and pediatric patients. This is a serious condition where small blood vessels in the liver become blocked, most often after a hematopoietic stem-cell transplant (HSCT).

Defitelio is specifically indicated when VOD is accompanied by kidney dysfunction or pulmonary (lung) dysfunction, making it a treatment for one of the most severe forms of this complication.
