Famotidine, Ibuprofen falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Berberine interacts with.
Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, taking berberine with drugs metabolized by CYP2C9 might increase drug levels and increase the risk of adverse effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Duexis combines two medicines: ibuprofen (an anti-inflammatory pain reliever) and famotidine (an acid reducer added to help protect your stomach). The ibuprofen part is the piece that matters here, because your liver clears it partly through an enzyme called CYP2C9. Early research suggests berberine may slow that enzyme down a bit, so ibuprofen could linger in your system a little longer than usual.
If that happens, you might notice more of ibuprofen's typical side effects, like stomach upset, heartburn, fluid retention, or, with heavier use, kidney strain. Keep in mind this is mostly theoretical and based on weak evidence, so it may not cause real problems for you. Still, since both can irritate the stomach, it's worth a quick chat with your pharmacist or doctor before combining them.

ANTICOAGULANT/ANTIPLATELET DRUGS

In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, berberine might have additive effects when used with anticoagulant and antiplatelet drugs and increase the risk of bleeding.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Duexis combines ibuprofen (an anti-inflammatory pain reliever) with famotidine (added to help protect your stomach). The piece that matters here is the ibuprofen. Like other NSAIDs, ibuprofen already nudges up your bleeding risk by making platelets a little less sticky and by irritating the stomach lining.
Lab and animal studies hint that berberine may also slow platelets from clumping. Stacked together, the theory is they could add up and raise your chance of bruising, nosebleeds, or stomach bleeding (think dark or tarry stools). To be clear, this is only theoretical and not proven in people, so don't panic. Still, since both touch bleeding, it's worth a quick chat with your pharmacist or doctor before adding berberine.
