Animal research suggests that berberine can have hypotensive effects. Also, a clinical study suggests that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone.

Your combo pill, hydrochlorothiazide plus metoprolol, is already doing two jobs to bring your blood pressure down: the water pill helps your body shed extra fluid and salt, while the beta-blocker eases your heart rate and the force of each beat. Berberine may gently lower blood pressure on its own, so stacking it on top could nudge your numbers down a little further than expected.

For most people this is a mild, theoretical concern, and the evidence is fairly weak (mostly animal studies plus one trial using a different blood pressure drug). Still, watch for signs your pressure dropped too low: dizziness, lightheadedness when standing, unusual tiredness, or a slow pulse. Don't change anything on your own. Just check in with your pharmacist or doctor before adding berberine so they can keep an eye on your readings.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, use of berberine with drugs metabolized by CYP2D6 might increase drug levels and increase the risk of adverse effects.

This combo pill pairs a water pill (hydrochlorothiazide) with metoprolol, a beta-blocker that helps slow your heart and lower blood pressure. The metoprolol piece is the one to watch here. Your body clears it through a liver enzyme called CYP2D6, and berberine appears to slow that enzyme down. When it does, metoprolol can build up in your blood and act stronger than expected.

In real life that might show up as a slower heartbeat, dizziness, unusual tiredness, or lightheadedness when you stand up. To be fair, this is mostly based on lab work and early studies, so it is more of a theoretical concern than a proven problem. Still, before adding berberine, check with your pharmacist or doctor so they can keep an eye on your pulse and blood pressure.
