Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group Berberine interacts with.

Theoretically, berberine might have additive effects with antihypertensive drugs.
Animal research suggests that berberine can have hypotensive effects. Also, a clinical study suggests that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone.

Here's the simple version: iloprost (Ventavis) is a vasodilator, meaning it relaxes and widens your blood vessels to ease the pressure in your lungs. Berberine, on its own, may nudge blood pressure down a bit too. So the concern is that putting them together could stack those effects and drop your overall blood pressure more than expected.

If that happened, you might feel dizzy, lightheaded, faint, or get flushing or a headache, especially when you stand up quickly. I want to be honest, though: this is mostly theoretical, based on animal studies and one trial using a different blood pressure drug, not iloprost itself. It's not a reason to panic. Still, since you're managing PAH, please run it by your doctor or pharmacist before adding berberine.

In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, berberine might have additive effects when used with anticoagulant and antiplatelet drugs and increase the risk of bleeding.

Here's the gist with iloprost (Ventavis) and berberine. Iloprost relaxes blood vessels, but it also acts a lot like an antiplatelet drug, meaning it makes your platelets less sticky so blood clots a bit less easily. Berberine has been shown in lab and animal studies to do something similar, slowing platelet clumping. Stack the two together and you may, in theory, raise your chance of bleeding or bruising.

To be fair, this is mostly theoretical and based on test-tube and animal work, not solid human studies, so I don't want to alarm you. Still, watch for easy bruising, nosebleeds, or bleeding that's slow to stop. Before adding berberine, please run it by your pharmacist or doctor first.
