Pazopanib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the gist for Votrient (pazopanib). Your liver clears this cancer medicine largely through an enzyme called CYP3A4. Berberine appears to slow that same enzyme down a bit, so the worry is that pazopanib could build up higher than intended in your blood. With more drug hanging around, you may notice more of its typical side effects, things like high blood pressure, diarrhea, nausea, fatigue, or changes in liver tests. Pazopanib can also affect heart rhythm, so steadier levels really do matter here.

This is a theoretical concern based mostly on lab and early human data, not proof it will happen to you. Still, since pazopanib is a serious medicine with a tight safety window, please don't start berberine without first checking with your oncology team or pharmacist.

Theoretically, berberine might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, berberine might have additive effects when used with anticoagulant and antiplatelet drugs and increase the risk of bleeding.

Here's the gist for these two together. Berberine has shown, mostly in lab and animal studies, that it can make platelets (the cells that help your blood clot) a little less sticky. Pazopanib (Votrient) is a cancer medicine that already carries its own risk of bleeding, including nosebleeds, bruising more easily, and rarely more serious internal bleeding. Stacking berberine on top could, in theory, add to that bleeding tendency.

I want to be honest about the evidence here: this concern comes from test-tube and animal research, not strong human studies, so it's more of a caution than a proven problem. Still, since you're already on a drug that affects bleeding, please run berberine by your oncologist or pharmacist before starting it. Don't stop or change your Pazopanib on your own.

What Is Pazopanib Used For?

Brand name: Votrient

Pazopanib tablets — sold as the brand Votrient and as generic pazopanib — are used to treat two types of advanced cancer in adults: advanced renal cell carcinoma (RCC), the most common form of kidney cancer, and advanced soft tissue sarcoma (STS) in patients who have already received prior chemotherapy.

Important limitation: Pazopanib has not been shown to work in adipocytic soft tissue sarcoma (a subtype involving fat cells) or in gastrointestinal stromal tumors (GISTs), so it is not used for those cancers.
