Riociguat falls under ANTIHYPERTENSIVE DRUGS — the group Berberine interacts with.

Theoretically, berberine might have additive effects with antihypertensive drugs.
Animal research suggests that berberine can have hypotensive effects. Also, a clinical study suggests that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone.

Here's the thing to know about pairing berberine with riociguat (Adempas). Riociguat already works by relaxing blood vessels, which lowers blood pressure as part of how it treats your lung pressure. Berberine, on its own, may also nudge blood pressure downward. So the worry is an additive effect: the two together could drop your blood pressure more than either alone.

If that happens, you might feel dizzy, lightheaded, tired, or even faint, especially when standing up quickly. I'll be honest with you though, this concern comes mostly from animal studies and weak evidence, so it's more of a theoretical caution than a proven problem. Still, since low blood pressure is already a known riociguat side effect, please loop in your pharmacist or doctor before adding berberine.

Riociguat also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the gist for Adempas (riociguat). Your liver uses an enzyme called CYP3A4 to help clear this medicine from your body. Berberine appears to slow that enzyme down a bit, so riociguat could break down more slowly and build up to higher levels than usual.

Since riociguat relaxes blood vessels and lowers blood pressure, more of it in your system could mean more low blood pressure, dizziness, lightheadedness, headache, or feeling faint, especially when you stand up. The good news is this concern is mostly theoretical and based on lab and early research, not strong reports in real patients. Still, riociguat is a sensitive drug, so please don't start berberine without first checking with your pharmacist or doctor.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold as Adempas and under the generic name Riociguat — are used to treat adults with two types of pulmonary hypertension (high blood pressure in the lungs).

The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots in the lung arteries, either after surgery or when surgery is not an option. The second is pulmonary arterial hypertension (PAH), where the arteries carrying blood to the lungs are narrowed or blocked. In both conditions, riociguat helps improve exercise ability and functional status, and in PAH it also helps slow the disease from getting worse.
