Telmisartan falls under ANTIHYPERTENSIVE DRUGS — the group Berberine interacts with.

Theoretically, berberine might have additive effects with antihypertensive drugs.
Animal research suggests that berberine can have hypotensive effects. Also, a clinical study suggests that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone.

Here's the simple version: berberine may gently lower blood pressure on its own. Since telmisartan is already a blood pressure medication, stacking the two together could push your numbers down a little more than expected. That's not necessarily a bad thing, but if your blood pressure drops too low you might feel dizzy, lightheaded, tired, or off-balance, especially when standing up quickly.

I want to be honest about the evidence here. This concern comes mostly from animal studies and one small human trial using a different blood pressure drug, so it's more of a theoretical, additive effect than a proven problem with telmisartan specifically. Food amounts aren't the issue; concentrated supplement doses are. If you'd like to try berberine, check with your pharmacist or doctor first so they can keep an eye on your readings.

What Is Telmisartan Used For?

Brand name: Micardis

Telmisartan tablets — sold as Micardis and the generic telmisartan — treat high blood pressure (hypertension). Bringing blood pressure down lowers the risk of strokes and heart attacks.

Micardis and telmisartan tablets are also approved to reduce the risk of heart attack, stroke, or death from cardiovascular causes in patients 55 or older who are at high cardiovascular risk and cannot take ACE inhibitors. This use is for patients with a history of conditions such as coronary artery disease, peripheral arterial disease, stroke, TIA (mini-stroke), or high-risk diabetes with signs of organ damage.
