Canagliflozin falls under ANTIDIABETES DRUGS — the group Bilberry interacts with.

Theoretically, bilberry leaf or fruit extract may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal research suggests that bilberry leaf extract might have blood glucose-lowering activity. Also, one small clinical trial in patients with type 2 diabetes shows that taking bilberry fruit extract 470 mg as a single dose prior to an oral glucose tolerance test lowers plasma glucose levels when compared with placebo.

Here's the simple version: bilberry may have a mild blood-sugar-lowering effect of its own. Canagliflozin (Invokana) already works to bring your glucose down by helping your kidneys flush out extra sugar. So if you stack the two, the worry is that your blood sugar could dip a little lower than intended, which means more chances of hypoglycemia symptoms like shakiness, sweating, dizziness, or feeling foggy.

That said, the evidence here is thin. It comes mostly from animal studies and one tiny human trial, so this is more theoretical than proven. Eating bilberries as a food is unlikely to matter. If you want to use a concentrated bilberry supplement, just give your pharmacist or doctor a heads-up so they can keep an eye on your numbers.

About Canagliflozin

What Is Canagliflozin Used For?

Brand name: Invokana

Invokana (canagliflozin) is used alongside diet and exercise to improve blood sugar control in adults and children aged 10 and older with type 2 diabetes. It is not for type 1 diabetes.

In adults with type 2 diabetes, Invokana is also used to lower the risk of heart attack, stroke, and cardiovascular death in those with established cardiovascular disease, and to reduce the risk of serious kidney complications — including end-stage kidney disease, cardiovascular death, and hospitalization for heart failure — in those with diabetic kidney disease and high levels of protein in the urine.
