Theoretically, bilberry fruit extract might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro, animal, and clinical research suggest that anthocyanidin extracts from bilberry can inhibit platelet aggregation.

Here's the thing to know about pairing bilberry with piroxicam. Piroxicam is an NSAID, and like other drugs in that family it already nudges your platelets toward not sticking together, which is part of why NSAIDs can irritate the stomach and raise bleeding risk. Bilberry's anthocyanin compounds appear to do something similar, gently slowing platelet clumping.

Stack the two and you could, in theory, see a bit more bruising, nosebleeds, or stomach upset and bleeding. I want to be honest though: this concern comes mostly from lab and animal studies, not strong human proof, and bilberry as a food is unlikely to matter. Concentrated supplements are the bigger question. If you take piroxicam daily, just check with your pharmacist or doctor before adding bilberry capsules.
