Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with black cohosh and bendamustine (Bendeka): both can be tough on the liver, so the worry is stacking two liver stressors on top of each other. Bendamustine itself can raise liver enzymes, and black cohosh has been linked, in rare reports, to liver inflammation and even liver failure. Put them together and you may have a harder time telling which one is causing trouble if your liver numbers climb.

I'll be honest though, this is theoretical and based mostly on scattered case reports, not solid studies. Still, since you're already getting chemotherapy, I'd keep it simple and run any supplement past your oncology team first. They watch your liver labs closely and can tell you if black cohosh is worth the risk right now.

Nearly all of the entries, about 80, involve the same liver enzyme, CYP1A2, which helps process bendamustine.”
