Here's the thing to keep an eye on with this combination: it's not about black cohosh changing how felbamate works in your brain or how strongly it controls seizures. Instead, both of these can be tough on your liver. Felbamate already carries a known risk of liver injury, which is why your doctor monitors liver labs while you take it. Black cohosh has been tied, in rare anecdotal reports, to liver problems too. Stacking two liver stressors together is the worry.

To be fair, this is theoretical and based on weak, case-report evidence, not strong studies. Still, watch for things like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before combining them.

Black cohosh is a North American plant most often used to ease menopause symptoms like hot flashes, but the research is mixed and far from settled. About Felbamate

What Is Felbamate Used For?

Brand name: Felbatol

Felbamate is not a first-line seizure medication. It is used only when a person's epilepsy has not responded well enough to other treatments and the seizures are severe enough that the serious risks of this drug are considered acceptable.

In adults, felbamate can be used alone or alongside other antiepileptic drugs (AEDs) to treat partial seizures (seizures that start in one part of the brain), with or without spreading to become a full seizure. In children, felbamate is used as an add-on treatment for both partial and generalized seizures linked to Lennox-Gastaut syndrome, a severe form of childhood epilepsy. Felbatol is specifically labeled for these uses in both adults and children.
