Kazano combines two diabetes medicines, alogliptin and metformin, that work to lower your blood sugar. The concern here is pretty straightforward: piperine, the active part of black pepper, may have a mild blood-sugar-lowering effect of its own. If you stack that on top of your medication, your numbers could theoretically drift lower than expected, raising the chance of hypoglycemia, things like shakiness, sweating, dizziness, or feeling unusually hungry or tired.

I want to keep this in perspective, though. This idea comes from animal studies, not solid human data, and the everyday black pepper you sprinkle on food is very unlikely to cause trouble. Concentrated piperine supplements are the real question mark. If you take one, check your blood sugar a little more closely and run it by your pharmacist or doctor first.
