Black Pepper and Aspirin, Caffeine, Hydrocodone

These two interact in 4 distinct ways. Each one is explained separately below.

Why Are There 4 Interactions?

Black Pepper interacts with whole categories of medication. Aspirin, Caffeine, Hydrocodone falls into 4 of those categories, so it can interact with Black Pepper in 4 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

1 of 4

Why this applies to Aspirin, Caffeine, Hydrocodone

Aspirin, Caffeine, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Black Pepper interacts with.

Theoretically, black pepper might increase levels of drugs metabolized by CYP2D6.
In vitro research suggests that some constituents of black pepper inhibit CYP2D6. This has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the piece that matters in this combo product: hydrocodone is the ingredient handled by the CYP2D6 enzyme, which is the one lab studies suggest black pepper's piperine might slow down. Hydrocodone is partly turned into a stronger pain-relieving form by CYP2D6, so blocking that enzyme is a bit of a mixed bag. In theory it could let hydrocodone itself build up, which might mean more drowsiness, dizziness, constipation, or slowed breathing, especially at higher pepper-supplement doses.

Honestly, this is only a theoretical concern from test-tube research and hasn't been seen in real people. The sprinkle of pepper on your food is no worry. Concentrated piperine supplements are where I'd pause and check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 4

Aspirin, Caffeine, Hydrocodone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, black pepper might increase levels of drugs metabolized by CYP3A4.
In vitro research and pharmacokinetic simulation data suggest that piperine, a constituent of black pepper, as well as the pepper fruit seem to inhibit CYP3A4. This has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination pill pairs aspirin and caffeine with hydrocodone, an opioid pain reliever. The piece that matters here is the hydrocodone, which your liver processes partly through an enzyme called CYP3A4. Piperine, the active compound in concentrated black pepper supplements, may slow that enzyme down a bit. In theory, that could let more hydrocodone linger in your system, nudging up effects like drowsiness, dizziness, constipation, nausea, or slowed breathing.

I want to be honest, though: this is based only on lab and computer studies, not real patients, so it's a fairly weak signal. A sprinkle of pepper on dinner is no concern. If you take a piperine-rich supplement (often paired with turmeric), just mention it to your pharmacist or doctor before mixing.

ANTICOAGULANT/ANTIPLATELET DRUGS

3 of 4

Aspirin, Caffeine, Hydrocodone also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, black pepper might increase the risk of bleeding when taken with antiplatelet or anticoagulant drugs.
In vitro research shows that piperine, a constituent of black pepper, seems to inhibit platelet aggregation. This has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your combination product (Damason-P) contains aspirin, which already thins the blood a little by keeping platelets from clumping together. The concern here is that piperine, the active compound in black pepper, has shown in lab dish studies that it can also reduce platelet stickiness. Stack that on top of aspirin and, in theory, you might bruise more easily or bleed a bit longer from a cut.

I want to be honest though: this is purely theoretical, based on test-tube work, and it has never actually been reported in people. The sprinkle of pepper on your dinner is nothing to worry about. Concentrated piperine supplements are the only real question mark, so just run those by your pharmacist before adding them.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

4 of 4

Aspirin, Caffeine, Hydrocodone also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, black pepper might decrease levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that black pepper induces CYP1A2. This has not been reported in humans.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combo product (aspirin, caffeine, and hydrocodone) is best known for the caffeine piece when it comes to black pepper. Caffeine is cleared by an enzyme called CYP1A2, and lab studies hint that piperine, the active part of black pepper, might gently speed that enzyme up. In theory, that could clear caffeine a little faster, meaning the small caffeine boost in this medicine might fade slightly sooner.

Honestly, this is a minor, theory-only concern. It has only been seen in test-tube and animal studies, never in real people, and food amounts of pepper are very unlikely to matter. It also doesn't change how the aspirin or hydrocodone work. If you take concentrated piperine supplements, just mention it to your pharmacist.

About Black Pepper

What is Black Pepper?

Black Pepper
  • Flavoring and culinary use
  • Digestive complaints like bloating and indigestion
  • Boosting absorption of other supplements
  • Antioxidant and anti-inflammatory support
  • Cold and congestion remedies

Black pepper is a common kitchen spice that is generally safe in the amounts used in food. Its extract, piperine, is mostly added to supplements to help the body absorb other ingredients (like curcumin from turmeric). Strong human evidence for black pepper treating specific health conditions is limited, so use supplement-strength products with care and check with your pharmacist.
