Aspirin, Caffeine, Orphenadrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Black Pepper interacts with.
In vitro research and pharmacokinetic simulation data suggest that piperine, a constituent of black pepper, as well as the pepper fruit seem to inhibit CYP3A4. This has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill mixes aspirin (a pain reliever), caffeine, and orphenadrine (a muscle relaxer). Of these, orphenadrine is the piece that leans on CYP3A4 to be broken down. The idea here is that piperine, the active compound in black pepper, might slow that enzyme down a bit. In theory, that could let orphenadrine build up slightly, which might bump up its anticholinergic side effects like dry mouth, blurry vision, drowsiness, constipation, or a racing heart.
Here's the honest part: this is only seen in lab and computer-model studies, not in real people. The pinch of pepper you sprinkle on dinner is almost certainly fine. Concentrated piperine supplements are the bigger question mark, so just run those by your pharmacist or doctor first.

ANTICOAGULANT/ANTIPLATELET DRUGS

In vitro research shows that piperine, a constituent of black pepper, seems to inhibit platelet aggregation. This has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product contains aspirin, which already thins the blood by keeping platelets from clumping together. The concern here is with that aspirin piece. In lab studies (test-tube only), piperine from black pepper also seemed to slow platelets a bit. Stacked on top of aspirin, the worry is a small theoretical bump in bleeding or bruising, things like nosebleeds, easy bruising, or stomach upset that lingers.
I want to be honest about the strength of this: it is theoretical and has never actually been reported in people. A sprinkle of pepper on your food is not the issue. Concentrated piperine supplements are where I would pause and check with your pharmacist or doctor first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro research suggests that black pepper induces CYP1A2. This has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece that matters for this combo pill: the caffeine in it is broken down by a liver enzyme called CYP1A2. Lab studies (in test tubes, not real people) hint that black pepper might speed that enzyme up a little. If that happened, your body could clear caffeine faster, meaning its pick-me-up effect could fade a bit sooner. The aspirin and orphenadrine parts aren't really affected by this.
Honestly, this is a very minor, theoretical concern. It has never been shown in humans, and the pepper you put on dinner is unlikely to matter at all. Concentrated piperine supplements are the only place I'd give it a second thought. If you take those daily, just mention it to your pharmacist for peace of mind.
