Here's the quick picture: your body uses a liver enzyme called CYP2D6 to help clear imipramine from your system. Lab studies hint that piperine, the active part of concentrated black pepper supplements, may slow that enzyme down a bit. If it does, a little more imipramine could linger in your blood, which can nudge up its usual side effects like drowsiness, dry mouth, constipation, blurry vision, dizziness on standing, or a faster heartbeat.

I want to be honest about the strength of this, though. It's only seen in test-tube research, not in real people, so it's theoretical. The pinch of pepper on your dinner is not a concern. If you're taking high-dose piperine or turmeric-piperine supplements, just mention it to your pharmacist or doctor first.

In vitro research and pharmacokinetic simulation data suggest that piperine, a constituent of black pepper, as well as the pepper fruit seem to inhibit CYP3A4. This has not been reported in humans.

Here's the gist for imipramine (Tofranil). Your body uses an enzyme called CYP1A2 to help clear this antidepressant. In lab dish studies, the piperine in black pepper seemed to speed that enzyme up. If that happened in real life, your body might clear imipramine a bit faster, which could leave less of it working and possibly weaken its benefit for depression or bedwetting.

The honest part: this has only been seen in test tubes, never in actual people, and it's rated a minor concern. The pinch of pepper on your dinner is almost certainly a non-issue. If you're taking concentrated piperine supplements, it's worth a quick chat with your pharmacist. Don't change your imipramine on your own.

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
