In vitro research suggests that some constituents of black pepper inhibit CYP2D6. This has not been reported in humans.

Here's the gist for tipranavir (Aptivus). Black pepper's piperine has been shown in lab dish studies to slow down an enzyme called CYP2D6. In theory, slowing that enzyme could let a drug it processes build up a bit, which might mean stronger effects or more side effects. With tipranavir that could look like nausea, diarrhea, stomach upset, or tiredness.
But here's the honest part: this is a theoretical concern only, seen in test tubes and never actually reported in people. Tipranavir is mainly handled by a different pathway (CYP3A), so CYP2D6 isn't its main route anyway. Everyday pepper on your food almost certainly won't matter. If you're considering concentrated piperine supplements, just run it by your pharmacist or HIV doctor first.

Here's the gist: your body uses a kind of cellular bouncer called P-glycoprotein to push certain drugs back out and limit how much gets absorbed. Tipranavir (Aptivus) is one of the medications this pump moves around. In lab studies, piperine (the active part of black pepper) seems to slow that pump down. In theory, that could let a little more tipranavir stay in your system, which might mean stronger effects and a higher chance of side effects like nausea, diarrhea, stomach upset, or changes in your liver tests.
I want to be honest, though: this is based only on lab and test-tube data, not real people, so it's more a 'good to know' than a red alert. The sprinkle of pepper on your dinner is very unlikely to matter. Concentrated piperine supplements are the bigger question. Since tipranavir is a key part of your HIV treatment, just run any black pepper or curcumin supplement past your pharmacist or doctor first.

In vitro research and pharmacokinetic simulation data suggest that piperine, a constituent of black pepper, as well as the pepper fruit seem to inhibit CYP3A4. This has not been reported in humans.

Here's the gist with tipranavir (Aptivus): your body relies on a liver enzyme called CYP3A4 to help process it. Piperine, the active part of black pepper, can slow that enzyme down in lab studies. If it did the same in people, slightly more tipranavir could linger in your blood, which might bump up side effects like nausea, diarrhea, tiredness, or stress on the liver.
The good news is this is theoretical. It comes from test-tube and computer modeling, not from real patients, so the concern is small. A sprinkle of pepper on your dinner is very unlikely to matter. Concentrated piperine or curcumin-with-pepper supplements are a different story, so run those by your pharmacist or doctor before starting.

Here's the situation with black pepper and tipranavir (Aptivus). Some early lab work suggests piperine, the active part of black pepper, might speed up an enzyme called CYP1A2. If a drug is cleared by that enzyme, revving it up could lower the drug's blood levels. For tipranavir, weaker levels could in theory mean less control of HIV over time.
Here's why I wouldn't lose sleep over it: this is only an in vitro (test tube) finding, it has never been shown in real people, and tipranavir is mainly handled by a different enzyme (CYP3A) and is taken with ritonavir to boost it. The pepper you sprinkle on dinner won't matter. If you take concentrated piperine supplements, just mention it to your pharmacist or doctor first.

What Is Tipranavir Used For?
Brand name: Aptivus
Aptivus (tipranavir) is used to treat HIV-1 infection in adults and in pediatric patients weighing at least 36 kg (about 79 lbs). It is specifically for people who have already tried other HIV treatments and whose virus has become resistant to more than one protease inhibitor. It is always taken together with a low dose of ritonavir and with other HIV medications.
Aptivus is not recommended for people who have never taken HIV treatment before (treatment-naïve patients). Using tipranavir alongside additional active HIV drugs improves the chances of it working well.
