Theoretically, taking Panax notoginseng might reduce the levels and clinical effects of CYP1A2 substrates.
Animal research shows that administering Panax notoginseng intravenously for 7 days before intraperitoneal injection of caffeine can decrease maximal blood levels of caffeine by 37%. This interaction was attributed to the ability of Panax notoginseng to increase the activity of CYP1A2.

Here's the idea behind this pairing. Lab and animal studies hint that Panax notoginseng may speed up an enzyme called CYP1A2, which can clear certain drugs out of the body a little faster. Tipranavir (Aptivus) is processed in part through this pathway, so in theory the herb could lower your tipranavir levels. With an HIV medicine, that matters, because weaker drug levels could let the virus rebound or, over time, encourage resistance.

That said, please don't let this scare you. This is a theoretical concern from in vitro and animal work, not from people taking these two together, so we really don't know how big the effect is in real life. Still, HIV therapy is too important to gamble with. Before adding this supplement, run it by your pharmacist or HIV provider first.

About Tipranavir

What Is Tipranavir Used For?

Brand name: Aptivus

Aptivus (tipranavir) is used to treat HIV-1 infection in adults and in pediatric patients weighing at least 36 kg (about 79 lbs). It is specifically for people who have already tried other HIV treatments and whose virus has become resistant to more than one protease inhibitor. It is always taken together with a low dose of ritonavir and with other HIV medications.

Aptivus is not recommended for people who have never taken HIV treatment before (treatment-naïve patients). Using tipranavir alongside additional active HIV drugs improves the chances of it working well.
