Ondansetron falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Panax Notoginseng interacts with.

Theoretically, taking Panax notoginseng might reduce the levels and clinical effects of CYP1A2 substrates.
Animal research shows that administering Panax notoginseng intravenously for 7 days before intraperitoneal injection of caffeine can decrease maximal blood levels of caffeine by 37%. This interaction was attributed to the ability of Panax notoginseng to increase the activity of CYP1A2.

Here's the idea behind this pairing. Your liver uses an enzyme called CYP1A2 to help clear ondansetron from your body. Lab and animal studies hint that Panax notoginseng may speed that enzyme up. If it does, your body could break down ondansetron a bit faster, leaving less of it around to do its job.

For you, that could mean the ondansetron works a little less well at calming nausea and vomiting. Keep in mind this is theoretical and only based on animal research (caffeine, not ondansetron), so we honestly don't know if it matters in real people. Ondansetron is not a prodrug, so it needs to stay active to work. If you're relying on Zofran during chemo or after surgery, check with your pharmacist or doctor before adding this herb.

About Ondansetron

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
