Perampanel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Panax Notoginseng interacts with.

Theoretically, taking Panax notoginseng might reduce the levels and clinical effects of CYP1A2 substrates.
Animal research shows that administering Panax notoginseng intravenously for 7 days before intraperitoneal injection of caffeine can decrease maximal blood levels of caffeine by 37%. This interaction was attributed to the ability of Panax notoginseng to increase the activity of CYP1A2.

Here's the gist for Fycompa (perampanel). Your body uses a liver enzyme called CYP1A2 to help process this seizure medication. Lab and animal studies hint that Panax notoginseng may speed up that enzyme, which could clear perampanel out of your system a little faster and leave less of it working. In practice, that might mean weaker seizure control, which is the real concern with any anticonvulsant.

I want to be honest that this is very early, theory-level evidence (it comes from animal data using caffeine, not from people taking perampanel), so we don't know how much it matters in real life. Perampanel isn't a prodrug, so the direction holds. Before adding this herb, please check with your pharmacist or doctor first.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
