Asenapine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Panax Notoginseng interacts with.

Theoretically, taking Panax notoginseng might reduce the levels and clinical effects of CYP1A2 substrates.
Animal research shows that administering Panax notoginseng intravenously for 7 days before intraperitoneal injection of caffeine can decrease maximal blood levels of caffeine by 37%. This interaction was attributed to the ability of Panax notoginseng to increase the activity of CYP1A2.

Here's the gist for Saphris (asenapine). Your liver uses an enzyme called CYP1A2 to help clear this medicine out. Panax notoginseng may speed that enzyme up, which in theory could move asenapine out of your system a little faster. If that happens, the drug could end up working less well, meaning your mood or schizophrenia symptoms might not stay as well controlled.

The honest part: this concern comes only from a lab and animal study (with caffeine, not asenapine), so it's more of a theoretical heads-up than a proven problem. Asenapine isn't a prodrug, so the direction here is reduced effect, not a buildup. Still, don't stop or change anything on your own. Just mention you're using this herb to your pharmacist or doctor so they can keep an eye on how you're doing.

What Is Asenapine Used For?

Brand names: Saphris, Secuado

Asenapine treats schizophrenia in adults and bipolar I disorder (manic or mixed episodes) in adults and in children ages 10 to 17. It can be used alone or alongside lithium or valproate in adults with bipolar I disorder.

The sublingual tablet forms (Saphris and the generic asenapine sublingual tablet) are approved for both schizophrenia and bipolar I disorder. Secuado, the skin patch, is approved only for schizophrenia in adults.
