Black Psyllium and Ertugliflozin

Here is the interaction we found between these two, with the evidence behind it.

Minor

ORAL DRUGS

Why this applies to Ertugliflozin

Ertugliflozin falls under ORAL DRUGS — the group Black Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with ertugliflozin (Steglatro) and black psyllium. Psyllium is a soluble fiber that turns into a thick gel when it hits water in your gut. That gel can sometimes grab onto a pill and slow or shift how much of it gets absorbed. The catch is that the effect is unpredictable, and we don't have solid studies on ertugliflozin specifically, so this is mostly a theoretical concern rated as minor.

If absorption did dip a little, you might notice slightly weaker blood sugar control. An easy fix keeps you in the clear: take your ertugliflozin first, then wait about 30 to 60 minutes before your psyllium. Keep an eye on your glucose readings, and check with your pharmacist or doctor if anything seems off.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

From our pharmacist’s overview on the Ertugliflozin page

“Ertugliflozin (Steglatro) works by making your kidneys flush out extra sugar, and nearly all 256 supplements on this page pull on that same lever, so this is a list to approach carefully rather than casually. Most entries either add to the glucose-lowering effect and raise the risk of a low, or, like niacin, push blood sugar the other…”
