Why this applies to Sotorasib

Sotorasib falls under ORAL DRUGS — the group Black Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.

Here's the simple version: black psyllium is mostly soluble fiber that turns into a thick gel in your gut. That gel can grab onto pills sitting in your digestive tract and change how much medicine actually gets absorbed. With Sotorasib (Lumakras), the concern is that the fiber could trap some of the dose, so a little less might make it into your bloodstream. Since sotorasib is working to slow your lung cancer, you don't want anything quietly reducing how much you absorb.

This is a theoretical, low-grade concern, not a proven problem with sotorasib specifically. A practical fix is timing: take your sotorasib first, then wait about 30 to 60 minutes before the psyllium. Please run this by your oncology pharmacist or doctor before adding fiber supplements.

Black psyllium is a soluble-fiber supplement made from the seeds of a Plantago plant, used mostly to ease constipation and support digestive health. It is best known and most studied for relieving constipation and may modestly help lower cholesterol, but it must always be taken with plenty of water. About Sotorasib

What Is Sotorasib Used For?

Brand name: Lumakras

Lumakras is used to treat adults with KRAS G12C-mutated locally advanced or metastatic non-small cell lung cancer (NSCLC) who have already received at least one prior cancer treatment. This use was approved under the FDA's accelerated approval pathway based on tumor response rates, and continued approval may depend on results from confirmatory trials.

Lumakras is also used in combination with panitumumab to treat adults with KRAS G12C-mutated metastatic colorectal cancer (mCRC) who have previously received certain chemotherapy regimens. In both cases, patients must first be confirmed to carry the KRAS G12C mutation using an FDA-approved test.
