Fiorinal with Codeine already packs two ingredients that slow down your brain and body: butalbital (a barbiturate) and codeine (an opioid). Both can leave you feeling drowsy, foggy, or lightheaded, and in higher amounts they can slow your breathing. The concern here is simple addition. Black seed has shown calming, sedative-type effects in lab and animal studies, so stacking it on top of these medicines could, in theory, make you sleepier than expected.

I want to be honest that this is theoretical and based only on animal research, not real patient cases, so don't panic. Still, since this combo is already strong, play it safe. If you notice extra grogginess or feel unsteady, check in with your pharmacist or doctor before continuing the supplement.

Theoretically, black seed may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research suggests that black seed extract can inhibit platelet aggregation and clotting, and increase bleeding time. In addition, decreased platelet counts have occurred in a human case report and in animal research.

The piece of Fiorinal with Codeine that matters here is the aspirin. Aspirin already thins the blood by keeping platelets from clumping together, which is exactly the same thing lab and animal studies suggest black seed may do. Stack the two and you could, in theory, nudge your bleeding risk a little higher. Realistically that might show up as easier bruising, nosebleeds, bleeding gums, or a cut that takes longer to stop.

I want to be honest about the evidence though: this is based mostly on test-tube and animal work plus a rare human report, so it is more of a caution than a proven problem. Food-amount black seed in cooking is unlikely to matter. Concentrated supplements are the bigger question, so check with your pharmacist or doctor before adding one.
