Candesartan Cilexetil, Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group Black Seed interacts with.
Clinical research suggests that black seed powder and oil might reduce blood pressure by 2-3 mmHg. In animal research, black seed modestly reduces blood pressure and concomitant use of black seed and amlodipine (Norvasc) or metoprolol (Lopressor) increased the blood pressure lowering effects of these drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: Atacand HCT already lowers your blood pressure two ways, since candesartan relaxes your blood vessels and the hydrochlorothiazide (a water pill) helps your body shed extra fluid. Black seed, on its own, may nudge blood pressure down a little more (studies suggest only about 2 to 3 mmHg). So stacking the two could add up and push your pressure lower than you want.
If that happens, you might feel dizzy or lightheaded, especially when standing up quickly, or feel unusually tired or weak. This is mostly theoretical and based on early, lower-quality research, and food or cooking amounts of black seed are unlikely to cause trouble. Concentrated supplement doses are the bigger question. Before starting black seed, check with your pharmacist or doctor, and keep an eye on your home blood pressure readings.

Black seed extract has shown diuretic effects in animals, which could theoretically increase potassium loss. This has not been shown in humans.

Here's the thing to keep in mind with Atacand HCT (candesartan plus hydrochlorothiazide). The hydrochlorothiazide part is a water pill that already pushes your body to lose some potassium. In animal studies, black seed also acted like a mild diuretic, so the worry is that piling the two together could drop your potassium a little further. Low potassium can show up as muscle cramps, weakness, tiredness, or an irregular heartbeat.
I want to be honest, though: this is a theoretical concern based only on lab and animal work, and it hasn't been shown in people. The candesartan side actually tends to hold onto potassium, which can balance things out. Food-amount black seed is unlikely to matter; concentrated supplements are less studied. If you take black seed regularly, just mention it to your pharmacist or doctor so they can check your potassium.
