Theoretically, taking black seed with antihypertensive drugs might increase the risk of hypotension.
Clinical research suggests that black seed powder and oil might reduce blood pressure by 2-3 mmHg. In animal research, black seed modestly reduces blood pressure and concomitant use of black seed and amlodipine (Norvasc) or metoprolol (Lopressor) increased the blood pressure lowering effects of these drugs.

Here's the simple picture: iloprost (Ventavis) is a vasodilator, meaning it relaxes and widens your blood vessels. Black seed seems to nudge blood pressure down a little too, by roughly 2 to 3 mmHg in studies. Put the two together and those small effects can stack, so your blood pressure could dip a bit lower than expected. With iloprost that might show up as dizziness, lightheadedness, headache, flushing, or feeling faint, especially when you stand up.

I want to be honest that this is mostly a theoretical concern built on lower-quality research, and a sprinkle of black seed as a kitchen spice is unlikely to matter. Concentrated supplement doses are the real question mark. Since iloprost treats a serious lung condition, please check with your pharmacist or doctor before adding black seed.

Theoretically, black seed may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research suggests that black seed extract can inhibit platelet aggregation and clotting, and increase bleeding time. In addition, decreased platelet counts have occurred in a human case report and in animal research.

Here's something worth knowing if you inhale iloprost (Ventavis) for pulmonary hypertension. Besides relaxing blood vessels, iloprost also makes your platelets a little less sticky, so it can gently nudge up your bleeding risk on its own. Black seed seems to do something similar. In lab and animal studies, its compounds slowed clotting, lengthened bleeding time, and in a few reports lowered platelet counts.

Stacked together, the theoretical worry is easier bruising, nosebleeds, bleeding gums, or cuts that are slow to stop. To be clear, this is based on early research, not strong human proof, and small culinary amounts of black seed in food are unlikely to matter. Still, before adding a concentrated black seed supplement, please check with your pharmacist or doctor first.
