Lesinurad falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Black Seed interacts with.

Theoretically, black seed might increase levels of drugs metabolized by CYP2C9.
In vitro research suggests that thymoquinone, a constituent of black seed, can decrease the metabolism of phenytoin by a mechanism possibly related to the inhibition of CYP2C9. The effect of black seed on CYP2C9 is unclear. This has not been shown in humans.

Here's the short version on pairing black seed with lesinurad (Zurampic). Your body clears lesinurad partly through a liver enzyme called CYP2C9. In test-tube studies, a compound in black seed (thymoquinone) seemed to slow that same enzyme down. If that happened in people, lesinurad could build up a bit and you might notice more of its known effects, like changes in kidney function or, rarely, the kidney problems lesinurad already carries a warning about.

I want to be honest, though: this is theoretical. It comes from lab work, not from people, and the real effect of black seed on this enzyme isn't clear. Food-amount black seed in cooking is unlikely to matter much; concentrated supplement doses are the bigger unknown. Before adding a black seed supplement, just run it by your pharmacist or doctor.

What Is Lesinurad Used For?

  • Lesinurad is used in combination with a xanthine oxidase inhibitor to treat hyperuricemia (high levels of uric acid) in people with gout (sudden attacks of redness, swelling, pain, and heat in one or more joints) whose disease is not controlled with their current medication. Lesinurad is in a class of medications called selective uric acid reabsorption inhibitors. It works by helping the kidneys to remove uric acid from the body.
