Here's the simple version: Perampanel (Fycompa) already causes drowsiness, and it's well known for making people feel sleepy, dizzy, or unsteady on their feet, especially when you first start it or go up in dose. Black seed isn't changing how your body breaks the medicine down here. Instead, the concern is that black seed may have its own mild calming, sedative-type effect, so the two together could pile on more tiredness than either alone.

I want to be honest about how solid this is: the worry comes only from lab and animal studies, not from people, so it's theoretical. Food-amount black seed in cooking is unlikely to matter. Concentrated supplement doses are less studied. If you'd like to try it, check with your pharmacist or doctor first, and watch for extra grogginess or balance problems.

Perampanel also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, black seed might increase levels of drugs metabolized by CYP2C9.
In vitro research suggests that thymoquinone, a constituent of black seed, can decrease the metabolism of phenytoin by a mechanism possibly related to the inhibition of CYP2C9. The effect of black seed on CYP2C9 is unclear. This has not been shown in humans.

Here's the gist with Fycompa (perampanel) and black seed. Perampanel is partly cleared by a liver enzyme called CYP2C9. In a lab dish, a compound in black seed (thymoquinone) seemed to slow that enzyme down. If the same thing happened in your body, perampanel could build up a bit and you might notice more of its usual side effects, like dizziness, drowsiness, trouble with balance, or mood and behavior changes.

I want to be straight with you, though: this is purely theoretical. It has only been seen in test-tube studies and never confirmed in people, so the real-world risk is probably small. A pinch of black seed as a kitchen spice is unlikely to matter. Concentrated supplements are less studied, so before adding one, please run it by your pharmacist or doctor.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
