Bog Labrador Tea and Acetaminophen, Caffeine, Dihydrocodeine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Acetaminophen, Caffeine, Dihydrocodeine

Acetaminophen, Caffeine, Dihydrocodeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bog Labrador Tea interacts with.

In vitro research shows that aqueous and ethanolic extracts of Bog Labrador tea leaves can increase the concentration of cytochrome P450 3A4 (CYP3A4) enzymes. Extracts obtained from different geographical regions in Quebec resulted in differing potency of CYP3A4 induction. Bog Labrador tea might increase the metabolism of CYP3A4 substrates.
Drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), amiodarone (Cordarone), ticagrelor (Brilinta), triazolam (Halcion), and others.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the wrinkle with this combination pill. Dihydrocodeine is a bit like codeine, it works partly as a prodrug, meaning your body has to convert it into stronger pain-relieving forms before you feel much benefit. Lab studies (just test-tube work, not people) suggest Bog Labrador tea may speed up a liver enzyme called CYP3A4. If that holds true in real life, it could clear this medicine a little faster, which might mean less pain relief than you'd expect.

Honestly, the evidence here is weak and the effect is uncertain, so I wouldn't panic over an occasional cup. Still, since pain control matters, it's worth a quick chat with your pharmacist or doctor before pairing the two regularly.

From our pharmacist’s overview on the Acetaminophen, Caffeine, Dihydrocodeine page

“Anything else that sedates you is the headline risk with this combination painkiller. Panlor and DHC Plus pair acetaminophen and caffeine with the opioid dihydrocodeine, and 78 of the 303 supplements on file, kava and even beer or wine among them, can deepen the opioid's drowsiness and, at the extreme, dangerously slow breathing.”

About Bog Labrador Tea

What is Bog Labrador Tea?

Bog Labrador Tea
  • Coughs and respiratory complaints
  • Digestive upset and diarrhea
  • Anxiety and stress
  • As a warm beverage
  • Aches and pains (traditional use)

Bog Labrador tea is a wetland shrub whose leaves have long been brewed as a traditional beverage and folk remedy, especially among Indigenous peoples of North America. There is very little human research to confirm any health benefit, and the plant is closely related to species that contain toxic compounds, so it should be used cautiously and in moderation. Pregnant or breastfeeding people should avoid it, and anyone considering it should talk with a pharmacist or doctor first.
