Bog Labrador Tea and Cobimetinib Fumarate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Cobimetinib Fumarate

Cobimetinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bog Labrador Tea interacts with.

In vitro research shows that aqueous and ethanolic extracts of Bog Labrador tea leaves can increase the concentration of cytochrome P450 3A4 (CYP3A4) enzymes. Extracts obtained from different geographical regions in Quebec resulted in differing potency of CYP3A4 induction. Bog Labrador tea might increase the metabolism of CYP3A4 substrates.
Drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), amiodarone (Cordarone), ticagrelor (Brilinta), triazolam (Halcion), and others.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Cotellic (cobimetinib): your body relies on a liver enzyme called CYP3A4 to break this medicine down. Lab studies suggest Bog Labrador tea may rev that enzyme up (induce it). If that happens in real life, your body could clear cobimetinib faster, leaving less in your bloodstream. For a cancer medicine, that's the concern, because lower levels could mean it works less well against the melanoma or histiocytic condition you're treating.

I want to be honest about the evidence here. This comes only from test-tube research, not from people, and potency varied a lot between plant samples. So it's a possible concern, not a proven one. Since cobimetinib is a serious medication, please don't add this tea without first checking with your oncologist or pharmacist.

From our pharmacist’s overview on the Cobimetinib Fumarate page

“If you are on cobimetinib (Cotellic) for melanoma, be strict about supplements, because this list carries real weight. The drug is cleared by the liver enzyme CYP3A4, and nearly all 138 flagged ingredients can push its blood level up or down through that pathway.”

About Bog Labrador Tea

What is Bog Labrador Tea?

Bog Labrador Tea
  • Coughs and respiratory complaints
  • Digestive upset and diarrhea
  • Anxiety and stress
  • As a warm beverage
  • Aches and pains (traditional use)

Bog Labrador tea is a wetland shrub whose leaves have long been brewed as a traditional beverage and folk remedy, especially among Indigenous peoples of North America. There is very little human research to confirm any health benefit, and the plant is closely related to species that contain toxic compounds, so it should be used cautiously and in moderation. Pregnant or breastfeeding people should avoid it, and anyone considering it should talk with a pharmacist or doctor first.
