Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Boldo interacts with.

Theoretically, taking boldo with anticoagulant/antiplatelet drugs might increase the risk of bleeding.
Animal and in vitro research shows that boldine, a constituent of boldo, has antiplatelet activity. In one case report, an adult taking a combination of boldo and fenugreek with warfarin experienced an increase in international normalized ratio (INR); however, it is unclear if this effect was due to boldo, fenugreek, the combination, or another factor.

Here's the thing to know about alteplase (tPA): it's a powerful clot-busting medicine given in the hospital through an IV, usually for a stroke, heart attack, or serious blood clot. By design, it already raises your bleeding risk while it's working. Boldo contains boldine, which lab and animal studies suggest may make platelets a little less sticky, so in theory stacking it on top of tPA could nudge your bleeding risk up further.

I want to be honest that this is very weak, mostly theoretical evidence, and tPA is an emergency drug your medical team controls closely. Still, since bleeding is the main worry with tPA, always tell your doctors about any boldo or other supplements you take. Never start or stop anything on your own.

Boldo is a South American shrub whose leaves are traditionally used for digestive and gallbladder complaints, but good human evidence is very limited. The leaves and oil contain ascaridole, which can be toxic to the liver, so only ascaridole-free products should be used and only for short periods. Talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.
