Boldo leaf contains ascaridole, a known liver toxin. Many cases of hepatotoxicity, including elevated liver transaminase levels and jaundice, have been reported in patients taking boldo.

Here's the concern when you pair Boldo with nitrofurantoin. Boldo leaf contains a compound called ascaridole that can be hard on the liver, and nitrofurantoin itself, though usually well tolerated, occasionally irritates the liver too. Putting two things together that each stress the liver may add up, theoretically raising your risk of liver trouble. Warning signs would include yellowing of the skin or eyes, dark urine, belly pain, or unusual fatigue.

I'll be honest, this is based on scattered case reports rather than strong studies, so it's a cautious heads-up, not a certainty. Since your UTI course is short, the bigger move is simply to skip Boldo while you're on the antibiotic. Please check with your pharmacist or doctor first.

Boldo is a South American shrub whose leaves are traditionally used for digestive and gallbladder complaints, but good human evidence is very limited. The leaves and oil contain ascaridole, which can be toxic to the liver, so only ascaridole-free products should be used and only for short periods. Talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.

What Is Nitrofurantoin Used For?

Brand names: Macrodantin, Furadantin, Furatoin, Macrobid

Nitrofurantoin treats urinary tract infections (UTIs) caused by specific susceptible bacteria. The capsule forms (including Macrodantin and nitrofurantoin macrocrystals capsules) are indicated for UTIs due to Escherichia coli, enterococci, Staphylococcus aureus, and certain strains of Klebsiella and Enterobacter species. Nitrofurantoin capsules (monohydrate/macrocrystals) are indicated only for acute uncomplicated UTIs (acute cystitis) caused by E. coli or Staphylococcus saprophyticus.

Nitrofurantoin oral suspension (Furadantin) is indicated for adults and children 1 month of age and older for UTIs caused by susceptible strains of E. coli, Enterococcus species, S. aureus, Klebsiella species, and Enterobacter species. No form of nitrofurantoin is approved for kidney infections (pyelonephritis) or infections in the tissue surrounding the kidneys (perinephric abscesses).
