Theoretically, taking boldo with anticoagulant/antiplatelet drugs might increase the risk of bleeding.
Animal and in vitro research shows that boldine, a constituent of boldo, has antiplatelet activity. In one case report, an adult taking a combination of boldo and fenugreek with warfarin experienced an increase in international normalized ratio (INR); however, it is unclear if this effect was due to boldo, fenugreek, the combination, or another factor.

Here's the simple version: Pradaxa (dabigatran) is already a blood thinner, and Boldo may have a mild blood-thinning quality of its own. Lab and animal studies suggest one of its compounds, boldine, can keep platelets from clumping. So if you combine the two, the worry is that they could add together and tip you toward more bleeding than the Pradaxa alone.

I want to be honest about how solid this is. The evidence here is thin, mostly test-tube and animal work plus one murky case report. It's a theoretical concern, not a proven danger. Still, with a blood thinner I'd rather you be cautious. Watch for things like easy bruising, nosebleeds, bleeding gums, pink or dark urine, or black stools. Please don't start or stop anything on your own. Run Boldo past your pharmacist or doctor first.

Boldo is a South American shrub whose leaves are traditionally used for digestive and gallbladder complaints, but good human evidence is very limited. The leaves and oil contain ascaridole, which can be toxic to the liver, so only ascaridole-free products should be used and only for short periods. Talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.

About Dabigatran

What Is Dabigatran Used For?

Brand names: Pradaxa, Pradax

Dabigatran etexilate capsules are used in adults to reduce the risk of stroke and dangerous blood clots in people with non-valvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). They are also used to treat deep venous thrombosis (DVT — a blood clot in a deep vein) and pulmonary embolism (PE — a blood clot in the lungs), to lower the risk of those clots coming back, and to prevent DVT and PE in adults who have had hip replacement surgery.

In children ages 8 to under 18, dabigatran etexilate capsules are used to treat venous thromboembolic events (VTE — blood clots in the veins) and to reduce the risk of those clots recurring. Pradaxa Oral Pellets — a separate formulation — are indicated for the treatment and prevention of recurrent VTE in younger children ages 3 months to under 12 years.
