Animal research shows that calamus is a CNS depressant and increases gamma-aminobutyric acid levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Bellergal-S contains phenobarbital, a barbiturate that calms the nervous system. The concern here is simple overlap: calamus appears to act as a mild nervous-system depressant in animal studies, partly by raising GABA, the same kind of calming signal phenobarbital works on. Stacking the two could, in theory, deepen the drowsiness, leaving you more sedated, foggy, dizzy, or slow to react than expected.
I'll be honest, this is a theoretical caution based on lab and animal data, not strong human studies. Even so, calamus has its own safety baggage and is generally best avoided. Please don't change anything on your own. Run it by your pharmacist or doctor first so they can weigh it with your full picture.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that calamus extract inhibits CYP3A4 enzyme.

Here's the gist with Bellergal-S (a combination of belladonna, ergotamine, and phenobarbital). Calamus has been shown in test-tube studies to slow down a liver enzyme called CYP3A4, which helps your body clear the ergotamine part of this medicine. If that breakdown slows, ergotamine could build up. That matters because ergotamine has a narrow safety window, and too much can cause cold or numb fingers and toes, leg cramps, nausea, or tingling from reduced blood flow.
That said, this is only a theoretical concern based on lab work, not real human cases, so I don't want to alarm you. Calamus is best avoided anyway for its own safety reasons. Please run this combination by your pharmacist or doctor before mixing them.

ANTICHOLINERGIC DRUGS

In vitro evidence shows that calamus can inhibit acetylcholinesterase (AChE).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Bellergal-S is a combination product, and one of its key players is belladonna, an anticholinergic that works by blocking acetylcholine activity. That is how it helps calm cramping, sweating, and other overactive nerve signals. Here is the catch: calamus has shown in lab studies that it can block the enzyme that breaks down acetylcholine, which leaves more of that chemical hanging around. Since calamus pushes in the opposite direction of belladonna, the concern is that it could blunt the belladonna's effect, so the medicine may not work as well.
I want to be honest that this is only based on test-tube data, not real human studies, so it is theoretical. Even so, given calamus's own safety worries, I would skip it and check with your pharmacist or doctor first.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.
