Cabotegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Calamus interacts with.
In vitro research shows that calamus extract inhibits CYP3A4 enzyme.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for the Cabenuva pairing: rilpivirine, one of the two medicines in your injection, is partly cleared by a liver enzyme called CYP3A4. In lab dish studies, calamus appears to slow that enzyme down. In theory, that could let rilpivirine build up a bit higher than usual, which might mean more of its known side effects, things like headache, trouble sleeping, mood changes, or dizziness.
I want to be honest, though: this is based only on test-tube research, not on people, so the real-world risk is unclear. The bigger worry with calamus is its own safety (it contains a compound the FDA banned from foods). I'd steer clear of it and check with your HIV provider or pharmacist before adding any supplement.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.

What Is Cabotegravir and Rilpivirine Injections Used For?
Brand name: Cabenuva
CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.
The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
