Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cauliflower interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with cauliflower and your Esbriet (pirfenidone). Your body clears this medicine mainly through a liver enzyme called CYP1A2. Cruciferous vegetables like cauliflower, broccoli, and cabbage can gently speed that enzyme up (studies suggest roughly a 14 to 27 percent bump). Since pirfenidone is the active drug itself and not a prodrug, a faster enzyme means it could be cleared a little quicker, which in theory might slightly weaken how well it works against your lung scarring.
The honest truth is this evidence is on the weaker side, and normal amounts of cauliflower in your meals are unlikely to cause a real problem. Keep eating your veggies. I'd just steer clear of concentrated cauliflower or cruciferous extract pills, and check with your pharmacist or doctor before adding any.
