Cauliflower and Grepafloxacin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Grepafloxacin

Grepafloxacin falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cauliflower interacts with.

Preliminary clinical evidence suggests that eating cruciferous vegetables, including broccoli, cauliflower, daikon radish sprouts, and cabbage, can increase cytochrome P450 1A2 (CYP1A2) activity by 14% to 27%. Theoretically, cauliflower might increase the clearance and decrease the effects of drugs metabolized by CYP1A2. Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline, zileuton (Zyflo), zolmitriptan (Zomig), and others.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with grepafloxacin (an older antibiotic that's actually been pulled from the market because of heart rhythm concerns). Part of how your body clears this drug runs through a liver enzyme called CYP1A2. Cruciferous veggies like cauliflower can nudge that enzyme to work a bit faster, by maybe 14% to 27% in early studies. In theory, that could speed up how quickly grepafloxacin is cleared, which might leave a little less in your system and slightly weaken the antibiotic's effect.

Honestly, this is a theoretical, modest effect, and normal food amounts of cauliflower are unlikely to cause a real problem. Since grepafloxacin is rarely used today, I'd just confirm anything with your pharmacist or doctor rather than worry about your dinner plate.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

About Cauliflower

What is Cauliflower?

  • General nutrition and a low-carb vegetable
  • Source of vitamin C and fiber
  • Antioxidant support
  • Digestive and gut health
  • Part of a heart-healthy diet

Cauliflower is a nutritious cruciferous vegetable that provides vitamin C, fiber, and plant compounds called glucosinolates. As a food it is healthy and safe for most people, but there is no strong proof that cauliflower supplements treat or prevent any specific disease. Enjoy it as part of a balanced diet rather than relying on pills.
