Cauliflower and Etonogestrel

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Etonogestrel

Etonogestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cauliflower interacts with.

Preliminary clinical evidence suggests that eating cruciferous vegetables, including broccoli, cauliflower, daikon radish sprouts, and cabbage, can increase cytochrome P450 1A2 (CYP1A2) activity by 14% to 27%. Theoretically, cauliflower might increase the clearance and decrease the effects of drugs metabolized by CYP1A2. Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline, zileuton (Zyflo), zolmitriptan (Zomig), and others.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the honest scoop. Etonogestrel is the progestin in your Implanon implant, and it's mostly handled by a different liver pathway (CYP3A4) rather than the CYP1A2 enzyme this cauliflower note is about. So the listed connection is a loose, theoretical one. Even for true CYP1A2 drugs, the evidence is that cruciferous veggies bump up that enzyme only a modest 14 to 27 percent.

In plain terms, eating cauliflower as part of your meals is very unlikely to weaken your birth control implant or change how it works. There's no realistic reason to skip this healthy vegetable. If you ever notice unusual spotting or have questions about your implant's effectiveness, check in with your doctor or pharmacist, but you don't need to worry about your dinner plate here.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

By severity: 2 major · 135 moderate · 38 minor.

From our pharmacist’s overview on the Etonogestrel page

“St. John's wort is the supplement to understand first if you use etonogestrel (Implanon), because it revs up the liver enzyme that clears the hormone and can leave you with less pregnancy protection than you think you have.”

About Cauliflower

What is Cauliflower?

  • General nutrition and a low-carb vegetable
  • Source of vitamin C and fiber
  • Antioxidant support
  • Digestive and gut health
  • Part of a heart-healthy diet

Cauliflower is a nutritious cruciferous vegetable that provides vitamin C, fiber, and plant compounds called glucosinolates. As a food it is healthy and safe for most people, but there is no strong proof that cauliflower supplements treat or prevent any specific disease. Enjoy it as part of a balanced diet rather than relying on pills.
