Cauliflower and Paracetamol

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Paracetamol

Paracetamol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cauliflower interacts with.

Preliminary clinical evidence suggests that eating cruciferous vegetables, including broccoli, cauliflower, daikon radish sprouts, and cabbage, can increase cytochrome P450 1A2 (CYP1A2) activity by 14% to 27%. Theoretically, cauliflower might increase the clearance and decrease the effects of drugs metabolized by CYP1A2. Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline, zileuton (Zyflo), zolmitriptan (Zomig), and others.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: cauliflower (and its cruciferous cousins like broccoli and cabbage) can nudge a liver enzyme called CYP1A2 to work a little faster. Paracetamol does use this pathway as one of several routes to be broken down, so in theory speeding it up could clear the drug a touch quicker and make a dose feel slightly less effective for pain or fever.

In real life, I wouldn't lose sleep over this. Paracetamol is mostly handled by other liver processes, the studies showed only a small bump in enzyme activity, and the evidence here is weak and largely theoretical. A normal plate of cauliflower at dinner is very unlikely to change how your Panadol works. If you ever feel it isn't controlling your pain, just check in with your pharmacist or doctor rather than adjusting anything yourself.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Paracetamol page

“Nothing on this list concerns me more than alcohol for people who take paracetamol (Panadol) regularly. Beer and wine both carry major ratings because paracetamol is processed by the liver, and drinking on top of it raises the odds of genuine liver injury.”

About Cauliflower

What is Cauliflower?

  • General nutrition and a low-carb vegetable
  • Source of vitamin C and fiber
  • Antioxidant support
  • Digestive and gut health
  • Part of a heart-healthy diet

Cauliflower is a nutritious cruciferous vegetable that provides vitamin C, fiber, and plant compounds called glucosinolates. As a food it is healthy and safe for most people, but there is no strong proof that cauliflower supplements treat or prevent any specific disease. Enjoy it as part of a balanced diet rather than relying on pills.
