Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chokeberry interacts with.

Theoretically, chokeberry might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that chokeberry inhibits CYP3A4. In humans, there is one case report of a drug interaction with trabectedin, a CYP3A4 substrate, which is hypothesized to have been caused by chokeberry inhibition of CYP3A4.

Here's the gist with your Evotaz (atazanavir plus cobicistat). Your liver uses an enzyme called CYP3A4 to help process atazanavir. In lab dishes, chokeberry seems to slow that enzyme down a bit. If that happened in your body, atazanavir levels could drift upward, which might mean more of its typical side effects, like yellowing of the eyes or skin, stomach upset, or changes in heart rhythm.

That said, I want to be straight with you: this is mostly theory. The real-world signal comes from a single case report with a different drug, not atazanavir. Eating chokeberry as juice or jam likely isn't a concern. Before you start a concentrated chokeberry supplement, run it past your pharmacist or HIV doctor first.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
