Why this applies to Irinotecan Hydrochloride
Irinotecan Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chokeberry interacts with.
In vitro research shows that chokeberry inhibits CYP3A4. In humans, there is one case report of a drug interaction with trabectedin, a CYP3A4 substrate, which is hypothesized to have been caused by chokeberry inhibition of CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thinking on chokeberry and your irinotecan (Onivyde). Irinotecan is partly cleared by a liver enzyme called CYP3A4. In lab dishes, chokeberry seems to slow that enzyme down, and there's a single real-world report where it may have done the same with a similar chemo drug. If chokeberry blocks CYP3A4 while you're on irinotecan, the drug could build up a bit more than expected, which could mean rougher side effects like severe diarrhea, low blood counts, nausea, or fatigue.
Now, I want to be honest: this is mostly theoretical and based on very thin evidence, and a little chokeberry juice or jam is unlikely to be the problem. Concentrated supplements are the bigger question mark. Since you're getting chemotherapy, please run any chokeberry supplement past your oncology team first.
