Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chokeberry interacts with.

Theoretically, chokeberry might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that chokeberry inhibits CYP3A4. In humans, there is one case report of a drug interaction with trabectedin, a CYP3A4 substrate, which is hypothesized to have been caused by chokeberry inhibition of CYP3A4.

Here's the gist for Adempas (riociguat): your liver uses an enzyme called CYP3A4 to help clear this drug from your body. Lab research suggests chokeberry may slow that enzyme down a bit. If that happened in real life, riociguat could build up higher than expected, which might push your blood pressure too low. That can leave you feeling dizzy, lightheaded, faint, or give you headaches or an upset stomach.

I want to be honest, though: this is mostly theory plus one case report with a different drug, so the evidence is thin. Food amounts like a little chokeberry jam or juice are unlikely to cause trouble. Concentrated supplements are the bigger question. Since riociguat needs careful dosing, just run it by your pharmacist or doctor first.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold under the brand names Adempas and Riociguat — are approved for adults with two types of high blood pressure in the lungs. The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots that have blocked or narrowed the arteries in the lungs. Riociguat is used when surgery has not fully fixed the problem, or when surgery is not an option.

The second is pulmonary arterial hypertension (PAH), a progressive disease where the arteries in the lungs become stiff and narrowed. In PAH, riociguat helps improve exercise ability, functional class (how well you can get around day-to-day), and slows disease worsening. It can be used alone or combined with other PAH medicines called endothelin receptor antagonists or prostanoids.
